Report to the employer
A report to an employer can only be provided with the explicit, written consent of the employee. This maintains strict professional confidentiality while supporting workplace integration.

Before your first visit
Check the referral letter from your attorney or employer for the specific appointment time and location.
Bring a valid form of identification (ID document, Passport or Driver's License).
Provide any previous medical or psychiatric reports related to your claim if they weren't already submitted.
Arrive 15 minutes early to complete any necessary administrative paperwork before your assessment begins.
